Fatal fall off loading dock

With a mix of forklift operations and pedestrian traffic, loading docks are inherently dangerous places if proper safety procedures aren’t followed.

Susan Henley of Columbia, SC was unloading a truck on a loading dock at Otis Spunkmeyer Bakery. She was later found pinned beneath the loading dock plate, her chest crushed under the weight. She was pronounced dead at the scene.

The Occupational Safety and Health Administration is investigating the incident.
