Crushed between steel rolls

Industrial equipment should always be locked and tagged out any time employees need to work near moving parts. Case in point.

Larry Kostelnik, 60, Westerville OH, worked in the receiving department of a cylinder plant. He became caught in a piece of industrial equipment and was crushed between two large rolls of steel.

Paramedics had to extricate him from the machine. He was rushed to the hospital where he died several hours later.

Kostelnik was a 19-year employee of the company.
