Crane collapses, two die, OSHA issues $168k fine

OSHA has issued a $168,000 fine for several safety violations following the April 2014 death of two employees of Mass Bay Electrical. OSHA said the violations were willful and that the company failed to properly train the workers and ensure the crane was properly operated.

Joseph L. Boyd III, 34, of Fall River, MA, and John Loughran, of Quincy, MA, 34, were killed when a 150-foot crane fell to the ground as they were working on power lines. They died at the scene.

OSHA investigated and said the crane was improperly operated; specifically, there were problems with center of gravity. Proper training and operation would have ensured the crane’s boom remained in place despite stresses.
