1,000 injuries in 36 months for company with 20,000 employees

OSHA has fined Ashley Furniture, Arcadia, WI, $1.7 million for 12 willful, 12 repeated and 14 serious violations and placed the company in its severe violators program. OSHA said the company has recorded 1,000 workplace injuries in the past three years. It has 20,000 workers in 30 locations nationwide.

The 12 willful violations were issued because of lockout and machine-guarding violations: The company failed to protect against unintentional startup of machinery during tooling and blade changes, and failed to provide adequate machine guards.

Of the 1,000 injuries, more than 100 were caused by working on machinery that didn’t have safety mechanisms in place. Serious violations included failure to train workers to work safely on machinery.
